August 15, 2025 – Toluca Lake, CA: Authorities have identified 51-year-old David Smith as the victim of a fatal collision that occurred early Monday morning on the Ventura (134) Freeway, according to the Los Angeles County Medical Examiner’s Office. The crash took place at about 12:11 a.m. on the westbound lanes near Cahuenga Boulevard. Two SUVs were involved in the accident, and one of the vehicles overturned onto its roof. Smith suffered fatal injuries in the wreck and was pronounced dead at the scene. The cause of the collision remains under investigation.
